Einen materiellen Fehler (BTM-Unfall vorletztes Jahr) kann man denke ich ausschliessen.
Es scheint als ob das Blocksystem einfach nicht funktioniert hat.
Ob von alleine oder wieder mal durch Einfluss von einem unfähigen Mitarbeiter, muss man abwarten.

Laut Amusement Business wurde California Screamin' 3 Monate vor dem Unfall - und das ohne Wissen von Intamin - modifiziert:

Disney modified coaster before collision

Last week's collision on the California Screamin' roller coaster at Disney's California Adventure occurred three months after Disney made major modifications to the braking system without consulting the ride's manufacturer, the Orange County Register reported Friday.

State records show that the Anaheim park changed two brakes on the steel coaster in April -- in the areas where two trains collided on July 29, sending 15 people to the hospital (AB, 7-29). The modifications were found to be safe by state investigators during a June 2 inspection, according to the newspaper report.

Yet Swiss manufacturer Intamin AG said Disney didn't consult the company before making the changes.
